How can someone be referred to the service?
An open referral system is in place - so anyone can refer or be referred to the service within their locality.
Can I refer a friend or a member of my family?
Only if that person agrees.
Where can I contact the Community Mental Health Team?
The Team can be contacted through the following telephone numbers: • Forfar/Kirriemuir: 01307 475239 • Arbroath/Carnoustie: 01241 437200 • Montrose/Brechin: 01674 832219
Do I have to attend an office to meet with someone?
A visit can be arranged in your own home or to a health centre or other suitable place where you can be seen with privacy and confidentiality.
Is this a confidential service and will information be passed to other agencies?
It is a confidential service - information will only be provided to other agencies with the agreement of the service user. However, if there are any concerns raised regarding vulnerable people appropriate action will be taken to safeguard their needs.
Who will I see in the Community Mental Health Team?
Once your referral has been assessed by the team they will allocate a Case Manager to meet with you.
How quickly will I be seen?
Your needs will be assessed and you will be seen as quickly as they warrant. If there are issues regarding safety or medical urgency these can be responded to on the day of referral. Less serious needs may, however, take a little longer.
Will I have to see lots of people?
No - you will be allocated one named worker to assist you.
